The links below take you to forms for Home Town Physical Therapy's forms to be print and completed prior to arriving at your first appointment.  The forms included in the registration packet enable us to learn about your condition and how it is affecting your life, understand your overall health, track the effectiveness of your rehabilitation, and accurately bill your insurance.

Patient Care Services:

Patient forms can be downloaded in a Microsoft Word format or they can be obtained through our office.  Printing the forms out ahead of time will make your initial evaluation check-in more efficient.  If you have questions while filling out the form please call us at 457-7678.

HIPPA:

You can review our HIPPA policy.  Even though we are a small clinic and are not held to the same standards as hospitals and other large health agencies - we have developed higher standards to ensure your safety of personal information.  If you have any questions regarding our policies, please contact our Privacy Officer at 457-7678.
